A country with no hospital
You would think in this day and age, that a necessity for any country would be at least one hospital, but there is one country that has no hospital.
That is if you class the Vatican as a country, not a city.
With the city being only a few miles from Rome, the Vatican has no need for a hospital. Also, other than the Swiss guard, most of the inhabitants are gentlemen of late middle age, you get few Cardinals and Popes under 50.
